Insurance:

Dr. Embrey has opted out of insurance health plans. However, the clinic will provide invoices that you may submit to your insurance for reimbursement via “out of network” coverage, which will reimburse some portion of the cost. Please call your carrier to confirm coverage of out of network costs. Most insurance companies will cover office or tele- visits, but not telephone calls or coordination of care (forms, letters, consultation with other providers, etc.). Costs for these services will be your responsibility.

Fees:

Dr. Embrey charges $400 per hour for adult, child, and adolescent services. These services include all in person and virtual visits as well as telephone calls or correspondence requiring more than 10 minutes. It also includes completion of forms, letters, and consultations with school officials. All fees are collected at the time of service via the credit card on file.

Sliding Scale:

Financial hardship should not be a barrier to receiving mental health or any medical care. Dr. Embrey offers a sliding scale for clients who are unable to pay the established rate in full. Please inquire for further details if this applies to you.

Cancelations:

If you are unable to keep your appointment, please call or email to cancel at least 48 hours in advance. If you cancel after this time, your visit will be considered a no show, and you will be charged 100% of the service fee.

Intake Forms:

When you schedule your first appointment, you will receive intake forms, which include your personal health information, privacy policies, consent forms, and credit card information. Please have these forms completed and returned prior to your first appointment.

Informed Decision Making:

Please keep in mind that as a patient of Dr. Embrey’s, in addition to conventional western medicine and treatments, you will also be able to choose from other complementary or alternative treatments, which are not normally utilized in conventional western allopathic medicine. These modalities may not have robust scientific evidence surrounding them the way conventional treatments like medications and psychotherapy do. However, evidence is accumulating, and Dr. Embrey will be happy to share with you the evidence that is available to guide you in making the right decisions for your individual treatment. The decision on which treatment modalities are best for you will ultimately be up to you. Please know that these treatments are meant to complement, not replace, conventional medical treatments.
